The Carrell Clinic
Dallas, Texas

OBJECTIVE:
The objective of this fellowship is to train a board-eligible or board-certified orthopaedic surgeon in the subtle nuances of physical examination, cost-effective diagnostic testing, conservative care, and surgery for disorders of the shoulder and elbow.  The fellowship is certified by the American Shoulder and Elbow Surgeons (ASES) and is staffed by surgeons fellowship-trained in shoulder and elbow surgery.

PHYSICIANS:
Sumant G. “Butch” Krishnan, M.D. (Fellowship Director)
Wayne Z. “Buz” Burkhead, M.D.

HOSPITALS: Baylor University Medical Center, Presbyterian Hospital of Dallas, Mary Shiels Hospital, Veterans Affairs Medical Center, Texas Scottish Rite Hospital

LENGTH: 1 academic year

MAXIMUM NUMBER OF FELLOWS: 2 fellows per year

STIPEND: Based on PGY-6 salary per ACGME guidelines

OVERVIEW:
The fellowship is primarily oriented toward clinical work and clinical research in adult shoulder and elbow problems in a high-volume tertiary referral practice.  Training will involve comprehensive exposure to both open and arthroscopic surgical techniques for all types of shoulder/elbow disorders.  Fellows will also be exposed to a high-volume of shoulder arthroplasty cases, both for reconstruction and for acute shoulder trauma at a Level 1 trauma center.

The fellow will primarily and independently staff the Shoulder and Elbow Service at the Veterans Affairs Medical center weekly.  This will involve both clinical and surgical experience and direct teaching of orthopaedic surgery residents from the University of Texas Southwestern Medical Center at Dallas.

In addition, the fellow will have the opportunity to participate in the Pediatric Shoulder and Elbow Service at Texas Scottish Rite Hospital.  This involves evaluation and surgical management of shoulder and elbow disorders in skeletally immature patients, usually for the sequelae of brachial plexus injury.

There is also opportunity for fellows interested in Sports Medicine to spend time with other physicians in the group who primarily care for local collegiate and professional sports teams (SMU Athletics, Dallas Cowboys, Dallas Stars, Texas Rangers).

ROTATIONS
The fellow will rotate with the Shoulder and Elbow Service physicians on a structured basis, both in the operating room and in the outpatient clinic.  One day per week will be designated an “academic” day for research

Rotations with other fellowship-trained Sports Medicine surgeons in the group will be individualized based on interest.

DIDACTIC CONFERENCES AND RESEARCH:
The fellow will participate in a weekly didactic teaching conference with the Shoulder and Elbow Service staff.  This will involve structured reviews of major topics in textbooks and current journals.  In addition, the fellow will organize and participate in monthly journal club reviews with the orthopaedic residents from UT Southwestern.

The fellow will be allocated one research day per week to pursue clinical and basic science work (either projects of independent design or those ongoing).  Access to biomechanical testing and cadaveric dissection is available at affiliated institutions.  A full-service arthroscopy lab is also available for hands-on research and training.

The fellow will be encouraged to complete two clinically relevant research projects during the fellowship period.

CALL SCHEDULE
The fellow will take call for the Shoulder and Elbow Service based on the group call schedule.  This averages two call days per month and one call weekend every two months.

OTHER OPPORTUNITIES:
The fellow has the opportunity to “moonlight” as an Orthopaedic Trauma Associate fellow in a Level 1 trauma center.
